BridgeBio’s BBP‑418 Nears First‑Ever Limb‑Girdle MD Treatment

BridgeBio Pharma (NASDAQ: BBIO) has received FDA acceptance and Priority Review for its investigational therapy BBP-418, which could become the first-ever approved treatment for limb-girdle muscular dystrophy type 2I/R9 (LGMD2I/R9). The FDA has set a target decision date of November 27, 2026.

Drug and Disease Background

BBP-418 is an oral small-molecule therapy designed to work as a glycosylation substrate that supplies extra substrate to the partially functioning FKRP enzyme. This enzyme adds sugar chains to alpha-dystroglycan protein, which helps stabilize muscle cells and may preserve muscle strength and function over time. LGMD2I/R9 is a genetic, progressive form of limb-girdle muscular dystrophy that weakens hip and shoulder muscles and can affect heart and lung function. Currently, no approved treatments exist for LGMD2I/R9 or any form of limb-girdle muscular dystrophy, so patients rely on supportive care like physical therapy and mobility aids.

Clinical Trial Results

The FDA application is supported by positive Phase 3 FORTIFY trial results, which showed BBP-418 met all pre-specified primary and secondary endpoints. Treated patients showed statistically significant and clinically meaningful improvements in ambulation and lung function at 12 months, while placebo participants showed expected disease progression. The safety profile was favorable with no new or unexpected safety signals.

Regulatory Status

BBP-418 has received Orphan Drug, Fast Track, and Rare Pediatric Disease designations from the FDA, plus Orphan Drug designation from the European Medicines Agency. BridgeBio is preparing for a potential U.S. launch in late 2026 or early 2027 and is engaging with European regulators for an expedited approval pathway in the EU.
